The question Jennifer Sweeton gets more than any other is some version of this: can I change that? Clinicians trained in the eight-phase model, working with a seven-year-old who will not sit still and track a hand, want to know how far they can move from the protocol before they are no longer doing EMDR.

Her answer starts with mechanism. If what makes the therapy work is bilateral stimulation helping a client desensitise to distressing material faster, then that is the part you hold onto, and the specific delivery is negotiable. Eye movements are one way. A toy drum the child is beating is another. Once you know which element is load bearing, you can be flexible with everything else and still be true to the model. That reasoning, rather than a list of permitted substitutions, is what the three hours are built on.

She works through ages two to four, five to seven, eight to ten, and eleven upward, while being clear these are guidelines rather than boundaries, since some four-year-olds belong in the next band and some seven-year-olds do not. She also covers modifications for older adults. EMDR is well supported for children with post-traumatic symptoms, attachment difficulties, dissociation and self-regulation problems, and it also reaches anger, depression and anxiety, and can be used to build confidence and self-esteem rather than only to reduce distress.
