You have a client who has tried everything. They can describe the pattern accurately, they know where it came from, and they still cannot hold a settled state for more than a few days. Insight has not moved it, and neither have the strategies.

When Carol Kershaw and Bill Wade were writing Brain Change Therapy, they went through the literature asking whether every school of therapy shared a common denominator. What kept appearing was state change. Change the client’s emotional state and the thinking and the behaviour become available to change, which is why the same intervention lands in one session and does nothing in another. They pair that with state-dependent memory and learning, the finding that what you take in while in one state is most reachable when you return to it, which has direct consequences for what a client can actually get to in your room.

Seven hours across six modules, from two clinicians who ran a neurofeedback and biofeedback clinic for thirty-five years alongside their practice, and who direct a Milton Erickson Institute. They start with the therapist rather than the client, which is unusual and is deliberate. Then the research on change, the internal imagery clients keep running against themselves, deconditioning and reconditioning old patterns, and two modules of practical regulation strategies you can use in session and hand to a client. They are explicit that self-regulation is not repression, and that the tools are meant to be kind rather than overwhelming.

  • Physiological regulation you can teach in a few minutes, including paced breathing and hand warming, drawn from their biofeedback work
  • Structured visualisation, including a three-screen sequence for moving a client from the current situation to an action to an end result
  • Managing intense feeling as a wave with a rise and a fall, and the shift from trying to eliminate a state to practising the one you want more often

Dr Wade is a psychotherapist and international trainer known for integrating the neuroscience of psychotherapy with practical hypnosis skills. He is co-director of the Milton Erickson Institute of Houston and practices in Houston, Texas, where he works with individuals and couples on anxiety, depression, panic, trauma, relationship enhancement, divorce recovery, parenting, and peak performance. Trained in psychiatry and religion at Union Theological Seminary in New York City, he brings a thoughtful, mind-body-spirit perspective to his clinical and teaching work.

Clinically, Dr Wade focuses on how targeted hypnotic interventions and experiential methods can help clients regulate arousal, shift rigid patterns, and build new responses. His work draws on current understandings of the brain in psychotherapy while staying grounded in what actually helps clients feel and function better. He is a regular faculty member for Milton H. Erickson Foundation events, including Erickson Congresses and specialty panels such as “Meditation and Hypnosis.” He also teaches experiential courses on hypnosis for trauma and post-traumatic stress, and has offered trainings on hypnosis and mood and anxiety difficulties.
