Sensorimotor Therapy Techniques for Trauma Treatment: Practical Skills
You ask the client what they notice in their body and they say nothing. Not tight, not tense, not anything. They are in the chair, they are answering your questions, and they are not really there.
Jennifer Sweeton’s argument is that three things have to be in place before trauma processing does what it is supposed to do. A working alliance. A client who can be in their own body. And a stress response they can shift, even slightly. Miss any of them and the protocol underperforms, and it will look like the protocol’s fault.
The section on the alliance is the part most clinicians have never been taught. Sweeton gives it a mechanism. Mirror neurons fire when you act and when you watch someone else act, and they sit in the emotional centres of the brain as well as the motor ones. Watch someone’s arm being patted and your own skin tells you it is not happening to you. With emotional mirroring there is no skin, which is why merging with a client goes so much further, why countertransference carries real information, and why the boundaries we argue about are the skin we build on purpose.
Then the body. Why telling a client to scan their body can go badly when the body was the site of what happened, and how to work inward from the external environment instead. What to reach for when a client reports feeling nothing at all. And finally the stress response, taught with the mechanism attached, so you know why the breath work matters rather than just that it does.
Six hours on the part of trauma treatment that happens before the trauma work. If your protocol keeps stalling in the same place, this is usually where the problem is.
You’ll walk away with
- A treatment sequence that says what to do before trauma processing and why the order changes the result
- A mechanism for the therapeutic alliance, so it becomes something you can work on rather than something you hope for
- The reason emotional merging with a client runs deeper than ordinary empathy, and how to use that to catch your own countertransference before it starts shaping the session
- Coordinated movement exercises, including mirroring, tapping in tandem and seating placement, for re-establishing contact with a client who has drifted out of the room
- Embedded relational mindfulness: naming what you see happening in a client’s body so they feel seen and start noticing it themselves
- Co-regulation: meeting a client’s physiology where it is, then shifting your own breathing and posture to bring theirs with you, without saying a word about it
- Why a body scan can backfire when the body was the site of the trauma, and how to titrate in through the external environment first
- Grounding exercises graded by intensity, including where to start with a client who reports feeling nothing at all
- A quick resource scan for locating safe or strong areas of the body, narrowed to a single finger if that is all there is to work with
- Stress-regulation techniques with the mechanism behind each: diaphragmatic breathing and the vagus nerve, progressive muscle relaxation held at low intensity so clients learn to catch tension early, autogenic training for blood flow, and the diving reflex for acute moments
